Sedated Grooming Consent Form

For the safety and comfort of pets who experience high stress, anxiety, or medical concerns during grooming, sedation may be recommended. Please complete the form below to give your consent for sedated grooming.

Your responses help our veterinary and grooming teams ensure your pet’s experience is handled with care, compassion, and close monitoring. If you have any concerns or questions before submitting, don’t hesitate to reach out—we’re here to help.

Fully sedating a pet for a grooming service is a medical procedure performed in our hospital under the direct supervision of our veterinarian and technicians. I, the undersigned owner or authorized agent of the owner, am the responsible party for the pet identified above. I understand the risks involved as listed below and grant my consent to sedate my pet for grooming.

Sedation

I have had an opportunity to discuss the risks of sedation with the attending doctor. I understand that Altitude Animal Hospital will perform a pre-sedation physical examination prior to sedating my pet. A staff member will be monitoring my pet at all times while under sedation in order to minimize associated risks. Although Altitude Animal Hospital will provide appropriate medical care to follow every reasonable precaution, I fully assume all risks involved. Should some unexpected life-saving emergency care be required, and the attending veterinarian be unable to reach me, this practice’s staff has my permission to provide such treatment and I agree to pay for all related fees. I will not hold the clinic and or staff of Altitude Animal Hospital responsible in the event of injury or death of my pet.

Finances

I understand that there is an additional fee for sedation and I assume the full financial responsibility for the sedation as well as the grooming.

Grooming Expectations

Grooming under sedation is a modified process as the patient is groomed entirely lying down and appearance expectations must be modified as well. A haircut and or clipping will oftentimes not be as polished as if the pet were standing. For both the safety of the patient and the staff, some patients may still require a muzzle as they are not entirely unconscious. A muzzled patient will have their face groomed to the best of the groomers ability within reasonable safety measures. Sedated grooming patients are not always able to be safely bathed depending on temperament and may be sent home without a bath.

Emergencies

Although accidents are very rare, there is a risk when dealing with animals. Grooming equipment is sharp, and although groomers use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In most cases this can happen when an animal is wiggling or moving around. Your pet’s safety and comfort are our number one priority. In the event of an emergency, I authorize this establishment to immediately seek professional veterinarian attention for my pet (at my expense). I understand that all attempts will be made to contact me in the event of an emergency. I will not hold this establishment responsible for any pre-existing health problems my pet might have.

Vaccinations

In order to keep every animal that enters our facility safe, Altitude Animal Hospital requires core vaccinations for any pet receiving grooming services. Dogs must have current vaccines of DA2PP, Bordetella, and Rabies. Cats must have current vaccines of FVRCP and Rabies. I understand that I must provide proof of vaccination for my pet to be groomed. If I am unable to provide proof of vaccination, my pet will be turned away and unable to receive grooming services until vaccination requirements are met.

Coat Condition

I understand that this establishment puts my pet’s comfort above all else. In the event that my pet’s coat is matted, I understand that the groomer may have to shave the mats out rather than perform a painful dematting procedure. I also understand that if my pet is severely matted, there is an increased risk for clipper burn or cuts to occur. I understand that all attempts will be made to prevent this, however in many extreme matt conditions, it is unavoidable. I also understand that matted pets take additional time to groom so there will be an additional fee added on to the regular grooming price if my pet’s coat is matted.

Flea/Tick

I understand that if during the grooming process, it is discovered that my pet has any external parasites such as fleas and/or ticks, I give permission for this establishment to treat this issue at an additional charge. I will not hold this establishment responsible for any reactions to the flea/tick treatment.

Cancellation Policy

I understand that if I need to change my appointment time or cancel, I must give at least 24-hour notice so that the appointment time can be made available to another client who is on the waiting list. Appointments canceled within the 24-hour window or no showed will be subject to a 50% cancellation fee. If more than two appointments are missed without giving notice, I will be required to provide a 50% deposit in order to schedule any future appointment.
